LAKEWOOD HS Summer Sports Programs...

ORIENTATION MEETING

(for new and incoming athletes grade 9-12)

 Friday, June 12       9am      Big Gym

Please make sure to bring the following items, by the first day of summer practice, in order to participate in summer sports programs at Lakewood High School.  These are requirements and guidelines established by the Long Beach Unified School District.  Not all are required of all sports.  Contact the individual coach of the sport you are interested in for more details or see the Lakewood High School School Loop website for more information.  Full details and forms will be available at the meeting.

* Completed EMERGENCY CARD (all sports)

* $5 or $7.50  for MYERS-STEVENS INSURANCE coverage (varies by sport, may be included in donation amount)

* "APPLICATION FOR PARTICIPATION IN..." form, completed and signed (required by some sports)

* DONATION to cover costs and expenses for summer practices, tournaments, etc. (varies by sport)
