Athletic Contract and Expected Behavior

Lakewood High School has a strict athletic code of conduct for behavior in the class, on campus and in the community. On the left you can download the following:

• Athletic Contract
• Parent/Guardian Code of Conduct

Both forms are signed during the athletic clearance process by each athlete and parent alike and each athlete and parent must sign in order for them to play athletics at Lakewood High School.
